Zero-depth vaginoplasty includes rearranging the tissues of the penis and scrotum to generate a vulva (sensate clitoris, clitoral hood, labia majora, labia minora) and a functioning urethra. A vaginal canal will not be created during a zero depth vaginoplasty. This treatment allows for a functional outward look that corresponds to a person’s gender identification. Zero depth vaginoplasty is an alternative for patients who do not want penetrative intercourse, do not want to maintain the long-term dilatation associated with vaginoplasty, or are unable to have full depth vaginoplasty due to their medical history. Hair removal is not required prior to zero depth vaginoplasty. The surgery usually takes 3-4 hours and is performed in one stage under general anesthesia.

The Procedure:  

The Zero Depth Vaginoplasty is a one-stage operation that can take up to three hours to complete. An orchiectomy, penectomy, labiaplasty, and cliteroplasty are all part of the treatment. If an orchiectomy has not already been performed, the spermatic cord will be tied off to the inguinal canal. However, feeling is kept through a piece of the glans of the penis and the neurovascular bundle that runs along the shaft of the penis, which always remain linked. The neurovascular bundle is positioned correctly anatomically, on top of the pubic bone, to form the clitoris and clitoral hood.   

The urethra is shortened, a working urethral hole is produced, and the urethra is repositioned to an acceptable female location. 

Recovery:  

The zero depth vaginoplasty surgery is performed in one of our affiliated hospitals. You will have two drains, a catheter, a sutured compression dressing, and a tape dressing after surgery. You need to come for follow-up for two weeks so our surgeon can keep track of your progress. Your drains, catheter, and dressings are removed after one week. Your anatomy will also be evaluated to ensure that you are comfortable and confident before leaving.   

The next follow-up appointment will be in a week to monitor your progress, and you may leave following this session (if traveling out of town). Every person’s rehabilitation journey will be unique.
